Letter asking for teacher's help :)

I am applying for exchange and need my japanese teacher's help to fill out an application form. I am trying to ask politely for their help :)

こんにちは_______せんせい、

お久しぶりです。来年の一月に私は大阪の関西外大という大学*に行く予定です。関西外大に行って勉強するように、日本語の先生から(私に)お手紙をいただきます。**

*You can also say: 関西外大に or 関西外国語大学に

**If you've already received the letter, you need to write お手紙をいただきました.

    If you will be able to receive the letter, it's better to write お手紙をいただくことになっています.

もうしこみしょがあるのですがけど、手伝って貰いえたいです。時間があれば手伝ってください。よろしくお願いいたします。

--> I would say: もうしこみしょがあるのですが、かきかたがわかりません(or other reazon why you need a help)。もしお時間があるなら手伝っていただけませんか。よろしくお願いいたします。

返事を待っています。 (a bit politer: お返事をお待ちしています。)
