Personally, I think there is little difference: you simply name; "Mary" or "Jane" or describe; the fastest or the one with longest legs ... in answer to such a question. In general, I would pick "which" if the runners were named, and "what" if they were not and thus needed describing. In short, "which" is more intention, and "what" is more extention.

When you have unlimited possibilities for an answer, use what, so what runner is the winner because there might be thousands of runner in a marathon. In the other way, you know there's a finite answer, like in the twin situation, so which twin is taller.
