Get out of these clothes. You’re sticking out like a sore thumb.
I understand sticking out as being striking somebody's eye, and sore thumb as a thumb that's sore. Is it an idiom?

Here . "You’re sticking out like a sore thumb" means that the clothes are not fashionable or are too different from what other people are wearing..

Yes it was a idiomatic expression means "to be easily noticed as different"
