if the 1st person is questioning the 2nd person (who is in the locality) then "is it your name?" is the correct one and suppose 1st person is talking to 2nd person about the 3rd person(who is not in the location) then "does it his name?" is the correct one

what do you want to ask? if you want to know someone's name then you ask: What is your name? and if you already know someone's name and want to check it, then you ask for example: Is your name John?
