Which is correct? on the right below vs on the right side below

I'd like to express that the blank for writing the name is on the right side far below a surgery consent form.

To me, either is OK.
If I said 'right side', I would probably say 'right hand side', but perhaps that is just personal preference.
If it is far below, I would probably say 'at the bottom', or perhaps be more explicit about where, e.g. 'at the bottom of page x', or 'below the declaration', or something like that.
