'I am short-sighted [myopic] and the extent [degree] of my short-sightedness(near-sightedness) is 400'
Myopic is more often used figuratively to mean 'short-sighted' when referring to a plan, policy, etc.

Avoid using "I am myopic" for the exact reason that Adrian gave.
"I have myopia" is more correct but as others have mentioned, short sightedness is the common expression.
