Preventive action:
For the prevention of similar situations an extra training related to the selection of the medications was conduct for employees. All incoming medications will be placed into boxes with vacant space in reserve in order to take out the components without damaging during the selection.
Conclusion:
During the selection of the order #99224 kit №1364908 with a broken tamper seal was detected. On the basis of the investigation’s results, it was determined that the cause of the seal's breakdown was mishandling with the kit and closely packed kits in the boxes for keeping. In accordance with investigation results it was determined to pack less quantity of kits in one box in order to selection medications without damage. An extra training SOP (FCS/RUS/SOP/027 "Distribution order processing") was assigned.
