It is used as a way of expressing "even if there's any other way possible".
"I'll have 3 drinks tonight, if I drink at all." (if I drink anything)
"Drink alcohol in moderation, if at all."
"We seldom see her, if at all."
"If at all possible, would you please go to the store for some lemonade?"
"Do you have five dollars?" "I have five cents, if I have any money at all."

Hello, this is a tricky concept. The sentence is not used much in the modern English language, if at all. However it often means, 'if there is a chance' and it appears after expressions of scarcity. Example:
Roberto hardly ever goes to the gym, if it all.
The expression 'if at all' in the above sentence reduces the small chance of Roberto going to the gym to almost zero.
Another example: More women are having fewer children, if at all.
