What's the difference between 'sign' and 'symptom'? More than 99 percent of all Alzheimer’s drugs have failed tests in the clinic, and the few that have made it to the market only improve some________ .Not a single medicine has been shown to slow the continuous progression of the disease. Why '______' should be 'symptom' instead of 'sign'?? What's the difference between the two words?

Sign is neutral and general. You can have a sign of something good or bad. It doesn't even have to be medical. Medical is negative and usually (Though not always) associated with health.

"Sign" is a neutral word. "Symptom" is an indication of a disease (or figuratively, a problem).
