How would you say "he" in Japanese if you know the person and you're with them? What if you didn't know the name?

"if you know the person" means you know the name of the person. Then use the name. If it is 山田さん, you can (or should) always call him "山田さん”. (Do not use the word "彼“. It is too risky for you).
