I know it sounds so simple and obvious, but you would be suprised and how rarely it actually happens My question is: Can I leave out the second "and"? Is there any difference?

Joshua is right. Also, you could say “you would be surprised AT how rarely it happens.” Maybe you’ve heard somebody say “surprised at” and thought that you heard “surprised and.”

Yes. The second “and” needs to be removed.
