umm...this is not easy to translate. The inferred meaning is something like "You don't deserve to be called a (medical) doctor." "Your skill/moral is not good enough as a doctor." More literal meaning is "Are you a doctor with that?" You know, some types of people are socially expected to be capable of doing something, behave in particular way, and have some quality. Men are expected to be manly, doctors are expected to have good moral, skill and knowledge, for example. 「それでも・・・・か?」is a strong statement to doubt that the person fulfill those expectations. It's not always dehumanization, but also used to encourage people in a bit harsh way.
