what's the difference between "medico" and doctor" in spanish?

A 'Médico' es a licenced in general medicine he/she went to the University. The "doctor" is a higher universitary grade like orthipedist, the specialist in ear, nose and throat. There are doctorate in lows, sciences. . . etc.

the both are the same but medico is more comom in spain that in southamerica and viceversa
