A table is a grid containing information. It might be part of a report. A form is used to collect information. A business might send a form to a customer for them to complete and return.

The first two images are tables! It's a way of showing information. In a table, we have columns, that go down, and rows, that go left to right. They are a way of showing data. The last two images are forms. Forms are a way to ask for and collect information. They have questions and topics which have a blank space to be answered in. Many forms may ask for your name and contact details. Forms can be used when going to the doctor and they need information about you. They can also be used when applying for a job or leaving feedback for a service.
