“Being flaky as he is, I wouldn’t be surprised if he cancel on us tonight”
or
“Flake as he is, I wouldn’t be surprised if he cancel on us tonight”?

Being flaky is ok because after the word being, you need and adjective.
Or
Flake that he is. Also ok because here you need a noun.
Also the end of your sentence is incorrect. ... I wouldn’t be surprised if he cancelled on us. You need the past in this phrase.
