'lord knows' literally translated means 'god knows'. This is just an idiomatic way of emphasising what you say after tha.
'lord knows' (that) dreams are hard to follow.
Dreams are just that. Dreams. Making them real, or 'following your dreams' is generally difficult.
But - don't let anyone tear them away.
Just because dreams are difficult to follow, doesn't mean that you should give up on them. Don't let someone destroy your dreams (by ripping them away from you)

Lord knows = the almighty creator
Dreams are hard to follow = what you wish for, as in job, family, money, etc. This specifically pertains to what you want in life
But don't let anyone = self explanatory
Tear them away = keep you away from accomplishing your goals
So, in other words: the so-called creator is aware of the fact that what you really want in life is difficult to get, yet you should not let anyone stop you from having what you want and are capable of getting.
