Hi Bambang! If there is only 1 document, then you would say "here is the document," but if there are many documents, then you need to say "here are the documents." As for "that I require" and "that required," are you giving the documents to someone else? If so, then I would say, "here are the documents that you needed," or "here is the document that you needed" (if there is only one document). Hope this helps!
