The bodies of the elderly are weak at this age and cannot be as active as they age, and therefore they believe that they are a burden on society. These things will have a negative impact on their health and mental state, and they can enter a stage of deep thinking, and worrying about what will happen in the future.

A topic is pretty much the same as a subject: what thing is the paragraph talking about? In this one, the elderly.
The main idea is more focused on what a paragraph is trying to communicate about their topic: in this case, the main idea is probably that being elderly can be a difficult and vulnerable time of life.

The main idea is that the perception of weakness leads to negative impacts. I believe it overstates the decrepitude of the elderly.
