Whats the difference between "co tam" and "jak tam"?

Seeing these two phrases more often and im struggling to differentiate them. They seem very similar in their use, could anybody assist?

Both mean almost the same. It’s like competing what’s up with how are you.

"Co tam" is basically "What's up"

As an example :
"Co tam u ciebie?" - Whats up with you?

"Jak tam" is more like "Hows it?" or better said "Hows..." but it can also be "How was"

Examples :
"Jak tam spotkanie?" - Hows the meeting (going)?
"Jak tam dzisiaj w szkole?" - How was it in school today?
