1. Ni xing shenme? you reply " wǒ xìng ..." where ... is your surname
and for
2.Ni jiao shenme mingzi? you reply " wǒ jiào ..." where ... is your name

“Ni xing shenme”means "What's your family name?"and "Ni jiao shenme mingzi" means "what's your name？"You can answer with"wo jiao(your name).

You can try to use Kanji,1. "Ni xing shenme?"change into "你姓什么？"and
"Ni jiao shenme mingzi?"in Chinese means "你叫什么名字？"
