There are different formal formats you can use. It depends on who the recipient is and the purpose of the letter. For the United States, this is the most common formal format that I see: http://writing.wisc.edu/Handbook/BusLetter_Block.html There is also this format too, but I cannot recall a time that I've seen this format used in letters that I have received in the mail.
