In conversation how is "sie" differentiated between all the possible meanings. Is it purely down to context during conversation and guessing. Sie= singular formal Sie= Plural formal sie= she

Yes, it unfortunately is. But I think one will notice when being addressed (you => Sie) and when somebody is talking about other people (she => sie, they => sie). "Hallo, wie geht es Ihnen (, Aaron)? Was machen Sie heute?" "Wie geht es Ihren Kindern? Was machen sie heute?" "Wie geht es Ihrer Schwester? Was macht sie heute?"
