I assume this is supposed to be a joke. Maybe I don't get it but it sounds like there may be some more information because they are mentioning "the rules" which are not explained. As far as "holding" goes, it can mean either to remove something from the dish (hold the mayo means no mayo) or hold it physically, like between the legs. It seems like he just wants a piece of toast with his coffee, because if she holds all the ingredients mentioned, toast is all that is left.

i don,t certainly now the mean. but i do know the chicken between the waiter,s leg is a dick!
