Does "not really"mean "not so much" or just mean "no"?
for example,--"Did you know them well?'' --"No,not really"
Does it mean I don't know or I don't know at all.

It depends on the situation.
For your example "Did you know them well?'' --"No,not really" This might mean that the person DID know them, but not well. So they may have met them before, or know of them but would not consider themselves close to them, but perhaps an acquaintance.
So if you rephrased the question and asked "Did you know them?" The person would most likely answer "yes, I did know them" ... as opposed to "Did you know them well?"
Sometimes "not really" may be used as a more polite way of saying no (because there is still room for agreement). For example, if you asked me "Do you want to help me with my homework?" and I answered "not really" Then I am telling you no, I don't want to help you....but if you REALLY need me to help, I would probably help you.

It can have either meaning, depending on context. For the example that you have given, Al is right - it means that you only know them a little (not really well).
But in some circumstances it can also mean 'in reality, no', so take care how you use the phrase.
