"The patient's symptoms were fever, dizziness, and head pain." You could also say "headache" instead of "head pain."

The sentence isn't parallel... To make it parallel, it would be, "The patient's symptoms were dizziness, headache, and a fever." Or, "The patient's symptoms were that his head hurt, he had a fever, and he was dizzy"... But, that's just too long and complicated to *work* conversationally... It sounds awkward and clumsy... I agree with Megume, but one could also substitute the word "included" for "were" to have the sentence sound better.
