1b. (1a is ungrammatical)
2a. (2b is fine but unusual)
3 is trickier.  3a is grammatical; 3b is ungrammatical, but is more commonly heard than 3a, largely thanks to Charles Dickens!
4 are both grammatical, but they mean different things.  Answer to 4b: Tokyo.  Answer to 4a: on the south coast of Honshu.
5a.  (5b is ungrammatical)
