Duolingo is a good website/app.

En Breve: A Concise Review of Spanish Grammar. 5th Edition.

This book helped me immensely. I hope it does the same for you.

Spanishdict has been absolutely awesome.  The conjugations and the example sentence they give for the forms have been extremely useful and elucidating.
