The pregnancy went well, but ultrasound at the week 24 found out the bilateral kidneys’ enlargement. Therefore, another examination was scheduled for week 28. In addition to enlargement of kidneys, an oligohydramnios (reduced amniotic fluid amount) was found during the check-up.
The ultrasound (normal-sized liver and enlarged kidneys filling the abdomen):
Question: Which pathological conditions can be detected prenatally by ultrasound examination?
Answer: The disease is called polycystic kidney disease and is congenital. The nephron is poorly connected to ureteral base, that result in polycystosis and functional part of renal parenchyma reduction. Two types of polycystic kidney disease can occur. First one, occurring here, is an autosomal recessive type and great number of small cysts (microcystosis) are present in kidneys. The defect is serious, kidneys’ failure happens prenatally. That failure leads to a reduction in urine production and amniotic fluid amount. A foetus is also compressed in the uterus which result in facial malformation (facies Potteri). The risk of death within first year is very high, up to 40 %. On the other hand, autosomal dominant type (macrocystical, “adult”) results in less serious nephron defect and complications happen in an elderly age. The macrocystical type is usually diagnosed accidentally in adulthood.
In the first scenario an interruption is advised. In this case, the interruption was done at the week 29. The foetus dissection proved the high cysts number in both kidneys and facies Potteri.
