Did you take your dinner? I'm wondering if the grammar is correct. The past tense of take is took right? It should be Did you took your dinner? but I think it doesn't sound good. Which one is correct or is there another way of asking?

The first one is correct. "Did you take your dinner?"
When making questions (and negations) in past tense, you need to use the auxiliary verb "to do", which is then in past tense, and the main verb is in bare infinitive form (form of infinitive without "to").

So you have: "It went well.", but "Did it go well?"

He did it.
Did he do it?
He didn't do it.

1. After do / does / did, we ALWAYS use the base form (infinitive without "to"):

Does he EAT?
Did he EAT?
She does not EAT.
She did not EAT.

I think that most Americans would NOT say "to take your dinner."

I think that it is  more natural to ask:

Have you eaten dinner yet?
Have you had dinner yet?
