What is a "proprietary clinic"? "proprietary: privately owned and managed and run as a profit-making organization" Is this the word as opposed to a "free clinic"?

Yes but we wouldn't use the word "proprietary" in that context. You would just hear it referred to as a "private clinic" or "private practice".

Yes, as opposed to one run by a local authority/council/government/public body.
