"był" z Polski (male form) - Mój Dziadek BYŁ z Polski
"była" z Polski (female form) - Moja Babcia BYŁA z Polski

yeah, I agree with Martyna:
was from = 'był/była z ...' (or sometimes 'jest z...')
Although the sentence 'My grandad was from Poland' can be translated simply 'Mój dziadek był Polakiem' - in Polish it is more natural and oftener form to say that somebody was from certain country.
