It's not quite clear. A is either asking if Rick's ex-wife is B's sister or if the step-son's ex-wife is B's sister. Neither interpretation really makes sense since the first interpretation would mean the ex-wife was B's aunt, and the second would mean the ex-wife was his cousin's wife.

A was confused as to who Rick be there with, B, confirmed great uncle Rick was the closer family member,his grandmother’s brother, Ricks Ex wife is just mentioned in passing...

In this section A thinks Rick's Ex-Wife is B's Sister when, in reality, Rick is B's Great Uncle. This means that Rick is really old, and already related to B, so there's no chance that Rick is married to B's sister (since it would be incest).
