to lean out = to bend your body OUT of something, for example a window. "Don't lean out of the window, you might fall!"
to lean forward = to bend your body FORWARD. "I couldn't understand what she was saying, so I leaned forward."
to tell off = to reprimand someone, to scold someone
I hope this helps. :) One more thing though, if you've used a dictionary, you've tried to "look it UP"

Lean out like leaning out the window, or leaning out of the car window.( please don't lean out of the car window)
Lean forward like leaning forward in your chair to get a better look
Tell off can be taken two ways
First way:- to tell someone off like to send them away, to tell them to go away.
Second way:- to throw shade, to throw insane insults
