She came through again when I was struggling to break away from a rigid fundamentalist religious group that had isolated me from loved ones and driven me from a loving relationship with God. In American English it means "supported" or "was there for me" "I was feeling really sick last week, as usual, my wife really came through for me" There is a famous American song called "rock and roll dreams come through" which in British English many people call "rock and roll dreams come true" as the phrase does not exist in British English.

To simplify it even more "she came through again' means she helped me again (to overcome another obstacle, another difficulty). Basically, she was there to help me again. When someone really cares about you, they will be 'there' for you.
