My interpretation is there is some very unpleasant truth that the author does not want to accept.
There are two phrases that are used in English:
"It sticks in my throat" - refers to some situation that is very unpleasant and hard to accept.
"A bitter pill to swallow" - refers to somewhat the same situation, except that here, it means that the situation must still be accepted, even if unpleasant.
