the most common reply is :你叫什么名字？
a more polite way is : 请问您尊姓大名？

The short and polite one is: 您贵姓？
Normal: 你叫什么名字？ or 你叫什么？or 你谁呀？
Polite: 请问您的名字是？or 请问您尊姓大名？or 请问您是谁？
