It's rather hard for a native speaker to explain it since we use it naturally so I will probably skip some things, but I think the general scheme would look like this.
Firstly, we use the following words as the introduction:
jeśli or jeżeli (uninflecting) = if (exacly the same meaning)
jeśli nie or jeżeli nie (uninflecting) = unless/f you don't
gdybym / gdybyś / gdyby [on/ona/ono] / gdybyśmy / gdybyście / gdyby [oni/one] = if; you need to adjust it to the person. [on/ona/ono] etc. is optional.
gdybym nie / (etc.) = if not
1st Conditional
If you do it, I will hug you.
In Polish, in the 1st Conditional we won't use gdybym - only jeśli / jeżeli. The comma is important.
Jeśli to zrobisz, przytulę cię.
Jeśli + Future, Future.
Some similar sentences:
Jeśli tego nie zrobisz, nie przytulę cię. Jeżeli to zrobisz, nie przytulę cię. Jeżeli tego nie zrobisz, przytulę cię. (If you need translation let me know)
You can also put Imperative in the 2nd part:
Jeśli pójdziesz do sklepu, kup mi marchewkę. = If you go to the shop, buy me a carrot.
Jeśli + Future, Imperative
Examples:
Jeżeli nie chcesz iść do szkoły, zostań w domu. = If you don't want to go to school, stay at home.
Jeżeli zrobisz to zadanie, dostaniesz dobrą ocenę. = If you do this excercise, you'll get a good mark.
Jeśli nie ugotujesz obiadu, zamówimy pizzę. = If you don't cook dinner, we'll order pizza.
You're free to change the order of the clauses (no comma in this case):
Zamówimy pizzę jeśli nie ugotujesz obiadu. = We'll order pizza if you don't cook dinner.

You wrote it wrong, should be "Nie umiem używać trybu warunkowego po polsku"
