Hello.Long time no see.I'm glad at you keep on learning Japanese :D
I think this is really unique expression of Japanese language!
お弁当ついてる means that a grain of rice is clinging to around someone's mouth.
That's why Doraemon licked his mouth around.

言いかえると、「顔にお米がついてるよ」"You have rice on your face"という意味です。
日本では顔にお米がついているとき、「(顔に)お弁当(米粒)つけてどこ行くの?」とか「お弁当(米粒)ついてるよ」という言い方をします^^ でも、地方によっては「そんなの聞いたことない」という人もいます。
