Your sentence "Mój dziadek był z Polski" "...was from Poland" It's the past form, so: "był" z Polski (male form) - Mój Dziadek BYŁ z Polski "była" z Polski (female form) - Moja Babcia BYŁA z Polski Addition: Now : "Is from Poland" jest z Polski (female and male form) - Moja Bacia/Mój dziadek JEST z Polski "dziadek"- grandfather and "babcia" - grandmother of course :)

first translation: 1. "mój dziadek był z Polski" second 2. "mój dziadek pochodził z Polski" second translation sounds better.

yeah, I agree with Martyna: was from = 'był/była z ...' (or sometimes 'jest z...') Although the sentence 'My grandad was from Poland' can be translated simply 'Mój dziadek był Polakiem' - in Polish it is more natural and oftener form to say that somebody was from certain country.
