It means that the speaker is making a (non physical) gesture of friendship, like when you hold out your hand ready to shake the other person's in order to greet him/her.

It means I want to be your friend. Shaking hands is a sign of friendship. Extending my hand to you means I welcome your friendship. It is an idiom, but is used everywhere in the English speaking world.
