How to ask: "What is you name?" in Japanese? (Please just native speaker)

There are a lot of translations depending on the formality, so I'd like to write the most simple and common ones for the learners. (They are formal ones, casual ones but they never sound impolite.)
(After saying your name) おなまえは?
(After saying your name) あなたのおなまえは?
おなまえをおしえてください。(formal)
おなまえをおしえてもらえますか。(formal)
なまえをおしえて!(casual)
なまえをおしえてくれる / おしえてもらえる?(casual)
Ganbare yo = がんばれよ but it's informal and you shouldn't use it depending on the situation, especially, to people who are older than you and whom you talk with for the first time. I would say がんばってください.
