In what situations do you usually use this phrase?
Is it the same meaning as "even now?"

Yes they are very similar we can say....
"Even to this day I still can't eat rice, since I saw a worm in my rice 10 years ago".
"Even now I still can't eat rice, since I saw a worm in my rice 10 years ago"

Yes, it's very similar. 'to this day' would usually be used to refer to things that started to happen, or were well known, a very long time ago - I would say at least decades, but more likely centuries ago.
