d) Tom is ill.
(how long/he/be/ill?) _How long has he been ill?_

For A you should say "How long has she been learning Italian?" since she's still learning.
For B you should choose "How long have you known Tom?"
Otherwise they're all good. Great work!

(how long/she/learn/Italian?) _How long has she learnt Italian?_ (This one needs to be HOW LONG HAS SHE BEEN LEARNING? It is an incomplete process and still going on now)
(when/she/begin/learning Italian?) _When did she begin learning Italian?_

(how long/you/know/Tom?) _How long do you know Tom?(WRONG)_ or How long have you known Tom? (GOOD)
(when/you/first/meet/Tom?) _When did you first meet Tom?_
