大変申し訳ございません。お知らせするのを失念しておりました。
=たいへんもうしわけございません。おしらせするのをしつねんしておりました。
=Taihen moushiwake gozaimasen. Oshirasesuru nowo shitsunenn shite orimashita.
= I am terribly sorry. (However,) I forgot to inform (you).
*失念する(しつねんする Shitsunenn suru) is another expression which means "forget".
