Yes, both are grammatically correct. However, "continue your efforts" sounds a little more like a translation to English than natural English.
I would say one of the following (examples):
1. "Keep pursuing your dream(s)!"
2. "Continue pursuing your dream(s)!"
3. "Keep trying to achieve your dream(s)!"
