Both can be correct. It depends on the context.
"She cooked a meal yesterday evening" means that yesterday evening, she cooked a meal herself.
"She was cooked a meal yesterday evening" means that yesterday evening, someone cooked a meal for her.

She cooked a meal yesterday evening is correct.
Because the second sentence is grammatically incorrect. The verb cooked does not need a helping verb.
Example: I am angry today. Yesterday I was angry. Angry is the main verb, but am/was is the helping verb. Some verbs need helping verbs and these are mainly verbs that are related to emotions.
Other examples include. I am sad. I am happy. I am glad. etc

If she was cooked, that would mean she would be eaten! I don't think that's what you mean. Some different ways of saying this (you can just go through the tenses):
She cooked
She was cooking
She had cooked
She bad been cooking
She did cook

She cooked a meal -
this is in the ACTIVE voice. It means that she did the action. In this case, she cooked the meal
She was cooked a meal -
this is the PASSIVE voice (to be + past participle). It means someone else did the action. Often the 'agent', the one doing that action, can be omitted if it is not necessary. For example, maybe her mother cooked the meal.
She was cooked a meal by her mother.
