You're reading an advanced piece of literature. Ernest Hemingway and other classic, famous novelists use English metaphorically and even make up their own phrases. A prime example is the famous Irish James Joyce whom even English people find difficult to understand. This sentence basically means the trucks revved up (ground here means 'grind themselves up' - heavy trucks starting up) 'heading out of it all' - heading out means leaving - leaving that place behind.
