You could say “how many people are before her?” or perhaps “where is she in the queue?” if you’re taking about a line/queue.

“Is she your first child?” “No, she is the second.”

You wouldn't usually say "THE" second, just second. The only situations in which I imagine this being used is some sort of contest, race, line, etc. Example: How is she doing in the marathon? She's in second.
