It depends on the style. The first one (with a space and no period) is more British, and perhaps a bit more modern. The last one (with a space and a period) is more American and perhaps a bit more traditional. The middle one (with no space) is wrong. Again, this is just a question of style, so check which style is appropriate for the place where your work will be printed.
