is this sentence grammatically correct?

I'm translating my graduation certificate, and I want to make sure it's correct grammatically and offically.. 


( Having considered the examination results of the medical school in November 2017, the University Council decided on 28 February 2017 to confer upon MOHAMED AHMED, the bachelor’s degree in Medicine and Surgery. Registered in the university with No.374 )
